Standard lenses are covered in full after a $25 copay when you use an EyeMed network provider. That includes single vision, bifocal, and trifocal lenses - the copay is the same for all three.

Lens upgrades like UV coating, polycarbonate, and anti-reflective treatment aren't covered in full, but EyeMed members get special fixed pricing on them at in-network locations - typically far below retail.
