A Multi-State Plan (MSP) was a type of ACA Marketplace plan offered under a contract with the U.S. Office of Personnel Management (OPM), the agency that runs the federal employee health program. The program was created by the Affordable Care Act so that at least two plans would be available in every state.
Can I buy a Multi-State Plan in 2027?
No. According to OPM, MSP options were offered through the Health Insurance Marketplace only from 2014 through 2018. There are no Multi-State Plan options for 2027. You may still see the term in older articles.
A Multi-State Plan is not a short-term plan
Multi-State Plans were full ACA plans that covered pre-existing conditions and qualified for premium tax credits. They were not short-term medical plans, which are a different, non-ACA product. What if I need coverage in more than one state?
- Blue Cross Blue Shield PPO plans: many BCBS PPO plans give you in-network access in other states through the BlueCard program. See The BlueCard PPO Program: How Your Blue Cross Card Works in Another State.
- Other PPO plans: some Marketplace and private PPO plans include a national network. Check the plan's provider directory before you enroll.
- HMO and EPO plans usually cover only emergencies outside their service area, so they are a poor fit if you split your time between states.
Networks vary by plan and state, so always confirm out-of-state coverage before you buy.
Better alternatives to a Multi-State Plan
If you wanted a Multi-State Plan for nationwide coverage, these options do the job today:
- Private PPO plans (Cigna Ultra): Cigna Ultra plans use the Cigna Open Access national PPO network, with no referrals required, a 12-month rate lock and ACA-compliant benefits. They are available in all 50 states through Independent Health Agents, with group pricing you will not find on the open market. Cigna Ultra is offered as a Patriot plan, and you enroll online through the Patriot plans portal on Corenroll; enter agent code RKIL0783 on the enrollment page to get our group pricing. Anthem BlueCard PPO plans are also available. Compare private PPO plans or see Cigna Ultra plan details.
- Short-term medical: lower premiums for healthy people who need to bridge a gap in coverage. Short-term plans do not cover pre-existing conditions, and how long you can keep one depends on your state. See What States Can I Get a Year-Long Short Term Health Plan In 2027?
