If you choose one of the Ameritas Plans, you have the freedom to use any dentist. You will have access to the Ameritas Dental Network for greater savings but will have coverage for any dentist you visit.
If your provider is not currently in the network, you can also nominate them to become part of the Ameritas network by completing a provider nomination form. Please understand that nominating a provider doesn’t guarantee they will become an active participant in the Ameritas Dental Network.
